What is hair transplantation?

It is a follicular distribution in which follicles are removed from occipital and temporal sites of the head (these sites are not involved in genetic and hormonal action). Later, these follicles are implanted in bald or alopecic areas. 

What are follicular units?

Human hair grows in small groups called follicular units. There are 1, 2, 3, 4 and 5-hair follicular units. We always count follicular units and at the end, we calculate transplanted hairs. See Follicular Units. 

Are 2400 follicular units the same as 2400 hairs?

No.

An expert surgeon obtains more hairs per follicular units, since that fact depends on his experience in extracting follicular units and on the density of patient´s donor site.

For us, 2 400 follicular units approximately correspond to 4 800-6 000 hairs, depending on the quantity of 3,4-hair follicular units that patient´s donor site has. That would be a good follicular density.

How many follicular units do I need?

Firstly, we should be realistic about aesthetic needs of each person.

Total amount of grafts for each person depends on baldness type and scale of the patient –for advanced baldness more follicular units will be needed.

For advanced baldness we would require more interventions, so in the case of patients who need more than one procedure in order to get their aims, Dr. de Freitas will reward them with the cost of the following surgeries. 

Does the follicle or transplanted hair have the same growth and features as normal hair?

Yes.

Transplanted follicle has the same genetic information as a normal hair, since this is patient´s hair and keeps same texture and growing pattern. 

Could anybody have a hair transplantation done?

No, unfortunately.

There are certain cases in which transplantation can´t be an option, so it is very important previous medical visit.

How will I know if I am a good candidate for the transplantation?

The first thing to do is to go to the medical centre, where your scalp, hair and baldness scale will be evaluated. Later, the doctor will say if you are a good candidate, or not.  

In general terms, if the person is healthy –he has no important diseases and has good donor site, he would be a potential candidate for hair transplantation. Although medical evaluation is essential in order to investigate changes responsible for hair loss and to say if surgery is the best method to follow. 

What is the advantage of extracting hair following FUE technique, compared to STRIP technique?

The main advantage is that FUE technique is the way follicular units are extracted and implanted. Follicles are extracted one by one by using surgical instruments whose diameter range is less than 1 mm. That way there won´t be visible scars and will let the patient have short hair. Also, FUE technique doesn´t need stitches or bandages, so postoperative period is quiet and not painful and it lets the patient recover faster. 

If you decide have hair transplantation done with FUE technique, what would the first step be?

The first step would be to choose a doctor who has wide experience in this area and to choose the place where he would carry out the procedure under appropriate conditions. 

 After that, you should have a medical visit so that you personal case can be evaluated and an adequate treatment is planned for you.  

Does my hair need to be cut for surgery?

We tell patients there is no need of having shaved or short hair, that is, they can have their normal hair length.  That way we can evaluate existing hair and covering level. Just before starting the procedure we shave areas we are going to extract hairs from. In order to do hair transplantation we usually shave the whole head. In exceptional cases of patients who have long hair, short strips are shaved so that long hair can hide them –same method for women. However, only the doctor can evaluate the best thing for you.

Is hair transplantation recommended only for men?

No.

In fact, there are many women who suffer from baldness.

Only if surgery is diagnosed, we would proceed the same way as men. For women who have long hair, small strips are shaved so that long hair can hide them.  The implantation would be in selected areas between native hairs. 

For hair transplantation following FUE technique, is hospitalization needed?

No.

The procedure is done in the operating room which is prepared for that kind of surgery. The patient enters and goes out the same day. 

Is general anesthesia used?

No.

We would use local anesthesia. The patient is awake all the time. 

Is the procedure painful?

In general the patient does not feel pain during the procedure and even after the operation.  Hardly ever the patient can show a light pain which disappears by taking analgesics.

How long does surgery take?

In general it takes from 5 to 8 hours, but it can change depending on the patient and extracted or implanted follicular units. The patient will have breaks and he will be able to eat or go to the toilet. 

What can I expect after the procedure and when would I be able to do routines?

In general it is recommended to do rest for 2 days after surgery. Even there is no contradiction for going back to routines. After 15 days usual physical activities can be practiced. See After Hair Transplantation.

Will people realise I had hair transplantation done?

No.

We work with the principle of naturalness –with details in the frontal implantation line which imitate natural shape of each person. See Implantation Drawing. 

Transplanted hair, is it permanent or could it fall out with time?

Transplanted hair comes from areas which are not subject to DHT action (Dihydrotestosterone, hormone responsible for hair loss), so it is permanent. It falls down and renew physiologically depending on each hair´s follicular cycle. 

Should I have special care with transplanted hair? Can I dye my hair?

It is recommended not to use chemical products for the first 45-60 days after the procedure. After that period, the patient can start again his normal hair care routines. Although it is important to highlight that the continuous use of chemical products on the hair can damage it and provoke hair falling out –even if they are transplanted or not. 

Will I have an artificial look, like wrist hair locks or in recipient site?

No.

Artificial aspect is due to old-fashioned methods in which hairs were implanted following straight lines (these lines do not exist in nature) and the use of 3-or-more-hair follicular units in the front line.  Nowadays there are techniques that allow to have aesthetic, satisfactory, natural and imperceptible results.

Could be other person´s hair be transplanted (for example close relatives)?

It is not recommended, since the patient should take immunosupressor in order not to have rejection. It doesn´t mean we can´t do it, because we are talking about aesthetic, and not vital procedures –this would be applicable for transplantation of organs which are important for patient´s life. 

Could hair from other parts of the body be transplanted, apart from head?

Yes, it could.

However we don´t recommend the only use of body hair because growth cycle and hair quality is different, so final result would be in danger. This kind of hair is used to provide density to areas where hair of the head, together with other hairs, has been transplanted. For that, body hair must be chosen depending on texture, colour and thickness so that it has the same features as hair of the head. 

Could hair of the head be transplanted to other parts of the body?

Yes, it is very common to transplant hair of the head to other parts of the body, such as beard, moustache, eyebrow, chest and groin. 

Are there any risks of rejecting transplanted hair?

No.

Autotransplantation or autograft is the surgical intervention in which receptor and donor are the same person, that is, graft can be taken and implanted in the same person. The great advantage of this kind of transplantatio is that there is no rejection.

Will I have scars?

No.

After cutting there is a healing process. In FUE technique those cuts are made with millimetric cylindrical instruments (less than 1 mm diameter range) so there are very small micro white dots (diameter range less than 0,5 mm) in areas where follicular units were extracted. These dots are almost invisible and white, so they allow the patient to have very short hair.

 

Nowadays we use MFU technique associated to FUE. It consists of looking for multiple units so we extract one and leave another one. That way there will be hair in this space and effect will be reduced. 

When will I see results?

Results are progressive.

Around 10th month, we can see almost the final result. After 12 months, almost 90% of transplanted hairs will have reached normal growth. See when my new hair will grow.
